Continuum of Care - Lead Agency
In addition to administering the Homeless Management Information System (HMIS) for our area and conducting the Annual Point-In-Time Survey (PIT) of homeless individuals, CCHC is the central intake agency for all homeless clients, as the Lead Agency (designed by HUD), the Coalition organizes the collaboration of 65 local agencies to to address the needs of clients and provide essential services.
A major priority of the Coalition is to avoid overlapping of services.
The Charlotte County Homeless Coalition works diligently in our community to build agency partnership agreements with other organizations to meet the mental health, substance abuse, trauma care, physical health, and other social service needs of our clients.
